• certificate and appendix to it (or copies certified by a notary);

• certificate (s) of the Ukrainian Center for Educational Quality Assessment (EIE);

• passport copy;

• a copy of the certificate of assignment of the identification code;

• 6 identical color photos of 3x4 cm size;

• for young men: military ticket (registration certificate);

• medical certificate on form 086 / y;

• documents that give the right to benefits (if any).
